The following form will help you identify where you need help in caring for your loved ones.  This form is not an answer to your problems.  It is a guideline to help you start evaluating your situation.  It is YOUR responsibility to evaluate the information and use it to your advantage, or discard the information completely.

1.      List all the needs in each area (physical, emotional, spiritual, social). You can add to this list as you go.

2.      List the money you have available to meet those needs.

3.      List the people you have available to meet those needs.

4.      List the resources you have that you are not using.

5.      Compare the list of needs in the first column with the resources available in the next three columns. In the last column list the areas you still need help.

After you identify where you need help you will be able to search for some way to fill those areas.

Some of the places you might find help are: Area Agency on Aging, aging resources, hospital or nursing homes, home social workers, legal services, and various other local services. Just call them, explain your situation, and ask them if they know of some place you can get the help you need. On the Web Resources and Links page I have listed some web sites that have information that might help you. Some have support groups, also.
